This webpage was created for JSE staff and parents. It was designed to be informative and instructional, providing you with many health related resources. All information forms and links regarding any health, immunizations, or medical concerns can be found through the links above. Usually all of our students see the nurse at one time or another during the school year. If your child is visiting the nurse often, I will contact you. NURSES NOTES :

  • Prevention of Flu & Cold Wash your hands frequently, and cover your mouth when you cough or sneeze.

  • Please view the Forms and Links Menu above for information regarding School-Wide Health Screenings.

  • The Mobile Dentist is at JSE during January, information will be sent home in Thursday Folders.

REMINDERS:

  • Kindergartners and all new students must have all immunizations complete and a copy of dates provided to the nurse. Please see immunization information below and in the Link Menu.

  • All medications given at school must have a consent form, please see below or access the Forms Menu and complete the Medication Form . Epi-pens and inhalers are included on this form.

Immunization Information

Indian law requires that students in all grades are required to meet the minimum immunization requirements. The immunization record must include the student's name and date of birth, the vaccine given and date of each immunization. Immunizations must be complete before school begins. Contact your physician or school nurse with questions concerning your child’s immunizations.

Kindergarten through 5th Grade Immunization Requirements:

  • 3 Hepatitis B

  • 2 Hepatitis A

  • 2 Varicella

  • 5 DTap (Diphtheria, Tetanus, and Pertusses)

  • 4 Polio

  • 2 MMR (Measles, Mumps, and Rubella)
